Diabetes Mellitus Flow Sheet Minimum Practice Recommendations (PDF) (Revised March 2006)

Eye Referral Form
An eye referral form has been produced that can be used by patients with diabetes, Primary Care Physicians, and eye care providers to ensure that the appropriate exams are taking place to check for diabetic retinopathy and to ensure that the eye exams are being properly documented. After much consultation and review, the eye referral form was printed as a pad and will be distributed to physicians in Westchester County for use with their patients with diabetes.
