Baseline Survey of Health Plan Programs and Benefits

There is clear value in the evaluation of quantitative performance data. There is also value in qualitative data about health plans and diabetes care. These descriptive data offer a snapshot of what it means to be a patient with diabetes and to be enrolled in a health plan. To collect this information, Taking on Diabetes developed a baseline survey of health plan programs, benefits, and services.

The survey asked general questions about the process of diabetes care in health plans. Specifically, the survey collected information for the following categories:

  • Community Outreach and External Reporting
  • The Collection of Clinical Data
  • Practice Guidelines
  • Clinician Reminders about Care for Specific Patients
  • Patient Reminders and Education
  • The Process of Diabetes Care
  • Patient Cost Sharing for Diabetes-Related Care

A survey is currently being developed to assess the longitudinal impact of this initialized results from this survey.
